Registrar (StR) in Medicine for the Elderly and
General Medicine
to the East Midlands
(South) Healthcare Workforce Deanery
Geriatric Training Programme.
The post is
approved for higher specialist training in
both Medicine for the Elderly and General
(Internal)
Medicine. The post holder will rotate among the three Hospitals of the University Hospitals of Leicester NHS Trust,
Leicester General Hospital,
Leicester Royal Infirmary, Glenfield Hospital and
other hospitals participating in the Specialist training programme, and
may include rotation to Pilgrim Hospital, Boston,
Kettering General
Hospital and Northampton General
Hospital, which are within the
scheme, or possibly elsewhere.
The
Postgraduate Dean has confirmed that this post has the required educational and
staffing approvals.
The East Midlands Healthcare workforce Deanery
Geriatric Specialist Training Programme is normally
for a period of 5 years with award of a CCT in
both Geriatric Medicine and General
(Internal)
Medicine (Level 2). Trainees may
also seek a one year extension (year 6) in Leicester
leading to recognition in the
sub-specialty of Stroke Medicine, usually through open
competition at year 5. The
possibility of combined training with Acute Medicine
is currently being explored.
The contract is subject to special confidential
review at the end of the first year
to ascertain educational attainment and
work progress and annually
thereafter.
CONTRACTED HOURS
The contracted
hours will depend on the rota at different
hospitals during therotation. But, all will fulfil the European Working
Time directives as stated.
The contracted
hours are subject to change dependent upon the stage of the training programme and
individual local hospitals circumstances and
service needs
UNIVERSITY HOSPITALS OF LEICESTER

The
University Hospitals of Leicester

for the management of the Trust.
The Trust was established on
1st April 2000 and
comprises Leicester Royal Infirmary,
The Glenfield Hospital and the Leicester

services to be placed on each of

of debate with the particular involvement of clinicians.
GENERAL
Leicestershire Health covers a total population of about 875,000 of which approximately 132,000
are sixty-five years of age and
over. The Leicestershire Elderly Medicine Service was formed in
1965 and since
that time modern units have been
established in the City of Leicester with back up services
in peripheral hospitals in Market Towns
in the County.
Integration between
General and
Elderly Medicine has occurred in all three of the teaching
hospitals of the University
Hospitals of Leicester NHS Trust (Leicester Royal Infirmary,
Leicester General Hospital
and Glenfield
Hospital).
The University
Academic Department in Medicine
for the Elderly is based at the Leicester
General
Hospital.
Other
Hospitals
The post holder will be expected to rotate to hospitals
participating in the East Midlands (south) Healthcare Workforce Deanery Geriatric Training Programme. These include
Northampton

and Pilgrim
Hospital, Boston. Depending on one’s rotation,
the post holder may need to visit
other community hospitals in Leicestershire including Sunnyside Hospital,
Hinckley; St Luke's, Market

RESEARCH
& AUDIT
Encouragement and
time will be given for research and audit activities.
Success in these fields are
important and
trainees will be monitored for evidence
of satisfactory progress prior to renewal
of contract. The Post holder will be expected to involve actively in
the ongoing
research & audit projects of our Departments
and to establish his/her own particular research & audit interest.
ADMINISTRATION
Involvement with the arrangement of rotas, clinical
meetings, outpatient letters, discharge summaries, and medical audit will be expected.
TEACHING
Medical student
teaching will be an integral
part of the post, as students from Leicester University
receive much of their clinical
teaching at UHL and other hospitals in
the rotation. The post holder will
be expected to participate in teaching of students
undertaking
the 7 weeks Elderly and Chronic Care block with teaching
encompasses Practice of Geriatric
Care including
geriatric giants, stroke,
rehabilitation, community care, discharge planning and general
neurology.
In
addition, there will be opportunities for MRCP teaching
and participation in
teaching sessions involving physiotherapy and
occupational therapy staff as well
as for the nursing staff. The
post holder will be expected to attend
relevant local, regional and national
clinical meetings.
Opportunity
to visit other departments of medicine for the elderly and
care agencies will also be provided.
STUDY
AND TRAINING
East Midlands
Healthcare workforce Deanery is
committed to the development of
postgraduate training programmes for both general
and higher professional training as laid down
by Colleges and Faculties, and by the Postgraduate Deans'
network. At local level, college/specialty tutors work
with unit postgraduate clinical tutors in
the supervision of these
programmes. Trainees
will be expected to take part in
these programmes, including National
StR Audit as well as attending appraisal,
feedback and counselling
sessions/professional reviews.
Study leave will form part of these educational
programmes and will be arranged in
conjunction with the appropriate tutor.
There is a postgraduate medical centre and
library at each of the three Leicester
hospitals. A librarian is available to obtain
on loan
books and journals
not available in the library.
Lectures on a wide range of topics are held regularly and all centres
are actively concerned in
vocational training of
general practitioners in
the area. There are many weekly postgraduate clinical
presentations
or lectures as well as academic meetings
in the university
department of medicine for the elderly which the appointee will be expected to participate in.
POSTGRADUATE
TRAINING
All post holders will spend a minimum
of 12 months and
a maximum of 18 months in hospitals outside of Leicestershire.
While in
Leicester, the rotation
is based on a period of 1 year in each hospital, although rotation between
trainers will occur at 6 monthly intervals. The order of rotation
will not be fixed but determined, by the Programme Director after taking into
account of the needs of the trainees
to receive training in

Supervision
and support in
research in the field of Geriatric
Medicine is available for trainees in Leicester.
However, it is expected that many
trainees will have completed a period
of research before entering the scheme.
The local research interests
at present lie in the fields of hypertension, stroke, syncope,
Parkinson
disease and also medical services
for the elderly.
Trainees
will be required to maintain a training record (log book) and
will be subject to regular competence
assessments, appraisals and
annual
review (ARCP) as expected from PMeTB (Postgraduate Medical and Training Board) before proceeding
to a further part of the rotation.
PILGRIM HOSPITAL,
BOSTON
Pilgrim Hospital
is a District General Hospital serving
South East Lincolnshire with a catchments
population approaching 200,000.
It has over 600 beds and is a
modern hospital, the first phase of
which was opened in 1970. There
are 186 General Medical Beds, 5
Haematology beds and a 7 bedded Coronary Care/Intensive Care Unit. The major acute specialities are all represented in
the hospital and there are full
facilities for the investigation of patients,
including
CT scanning.
The Pilgrim
Hospital is more than 20 miles from the other District
General
Hospital serving
Lincolnshire patients and
the range of medical emergencies is very broad.

Kettering General
Hospital NHS Trust is an Acute District
General
Hospital with 550
beds. It serves a population of approximately 300,000 and
provides all of the acute services that would be expected of a hospital of its
size.
There are 247
medical beds on 8 Integrated Medical wards, an
Admission Assessment Unit
(35 beds), Cardiac Suite (10 beds) and
ITU (6 beds). Geriatrics trainees are based on
the two Naseby Wards.

Kettering Hospital has a comprehensive
stroke service run by Dr Ayes which
includes a stroke ward,
rehabilitation services and rapid access neurovascular
clinics. There are weekly joint
specialist Parkinson’s Clinics

Specialist Nurse who follows patients
up in the community. Dr
Hubbard also has clinics for patients with falls and
syncope as well as more general geriatric medicine. Intermediate
care and rehabilitation are carried out in
the community by Dr Lakkappa and Dr Kodavali and
opportunities for the SpR to
participate in this will be

HOSPITAL
Northampton
General Hospital is a busy DGH serving the town
of Northampton
and the Southern half of the County of
Northamptonshire. It has 680 beds and
serves a population of 346,000, of
which 47,100 (14%) are over the age of 65 years.
Department of Medicine
for the Elderly
The
purpose built department, opened in
1999 and provides 54 rehabilitation beds, 16 stroke beds, a dedicated out-patient, physiotherapy, occupational
therapy and speech therapy
facilities.
The
department is based entirely on
the Northampton General
Hospital site and there are no
long stay beds.

The departments
of Elderly Medicine General Medicine
are fully integrated.
There are 18 General
Physicians with special interests arranged
into seven
firms (3 firms have 3 Consultants the others have 2). Each firm has a junior
team of a FY1, FY2, Senior Medical
SHO, and SPR.
The new
MMC arrangements
will result in the current medical juniors
being reconfigured
into 10 ST posts, 5 FTSTA posts, 1
ACCS post, 5 FY2 posts,2 GPVTS posts (as existing).
The hospital has a well-established night team consisting of Senior
SHO medicine or SPR Medicine, FY2medicine,
FY2surgery, FY1 and night nurse
practitioner
Patients
are admitted to the 26 bed Emergency
Admissions Unit
which takes admissions for all
specialties. An assessment bay is available for assessment of patients
referred by their GPs.
The admitting
Consultant
is available on EAU on the afternoon/evening of the take day and
the following morning. The admitting
firms care for these patient’s
throughout their stay. Fixed commitments on
the take day and post take day are
cancelled and
the Consultant
takes the GP calls 9am-5.00pm.
Post Graduate Education
Each Wednesday
afternoon
there is an academic session comprising
image of the week, case presentations, topic teaching
and coaching
for MRCP part 1 and 2.
On
Tuesday mornings
there is a departmental teaching programme, including a monthly
x-ray meeting.
Research
There is an
active research unit on site. The unit
runs a number
of national
and international
studies on all aspects of elderly
medicine in
liaison with the responsible physician.
Teaching
The SPR is expected to take part in teaching
medical students (from the Oxford and Leicester/Warwick medical schools) and MRCP candidates.
Training Programme
Wide experience
of acute general medicine and
elderly medicine is on offer. The
detail of the elderly medicine
programme is variable according to
the individual’s interests and
needs. All junior

appointment is subject to the Terms and Conditions of Service of Hospital Medical and Dental
Staff (England and
Wales)
as amended from time to time and also such other policies and
procedures as may be agreed by the LNR Deanery.
The successful candidate
although contracted to the LNR Deanery will work within
Leicestershire Health Trusts. As a consequence
he/she is expected to be aware of local policies and
procedures and take note of the standing orders and
financial
instructions
of the appropriate Trust.
Infection
Control
The prevention of hospital acquired infection is a vital concern for the Trust. Infections harm patients.
Infections
also harm the Trust. The post holder is required to ensure, as an
employee, that his/her work methods do not
endanger
other people or themselves.
All staff must be aware of infection
prevention
and control
policies and guidelines, and
follow them at all times. Any breach
of infection
control policies is a serious matter
and may result in disciplinary
action.
Medical
Examination
This appointment is subject to medical fitness
and the appointee
may be required to undergo a medical
examination
and chest x-ray.
Potential
applicants should be aware of the
Department of Health and GMC requirements
with regard to HIV/AIDS and
Hepatitis viruses. Candidates are advised that they will be asked at interview whether they are aware of anything
preventing
them from fulfilling their professional commitment
and this includes
health.
Annual
Leave
Specialist Registrars on the third or higher incremental points
of their pay scale shall be entitled
to leave at the rate of 32 days a year.
Specialist Registrars on the minimum,
1st or 2nd incremental points
of their pay scale shall be entitled
to leave at the rate of 27 days a year.
Study
Leave
This is granted in accordance
with the recommended allowance, subject to the maintenance of
the service and to prior formal
approval by the Medical Personnel Officer of the Hospital you are based at.
Residence
This is a non-resident
post, although the appointee will
be required to be resident when on-call
depending
on local circumstances.
Accommodation
These posts are designated
non-resident and
for details regarding hospital
accommodation or married
accommodation, which may be
available, enquiries should be made
to the appropriate Trust Accommodation
Officer at the base Hospital.
CRB Checks
This post requires a enhanced disclosure by enhanced criminal
records certificates as this post is regulated by the Protection of Children
Act 1999, The Criminal Justice
Courts Act 2000 and / or the Care
Standards Act 2000 and such other authority committed by statute.
Failure to disclose details if you are currently
/ or in the future are the subject
of police investigation / proceedings
which could result in a conviction,
caution or bind
over order is a disciplinary matter,
and may result in dismissal.
Notification
of Termination
of Employment
The
post holder is required to give a minimum
of three months' notice of termination of their employment. Only
in exceptional
circumstances will less than three months'
be allowed.
HEALTH
& SAFETY
The University Hospitals of Leicester NHS Trust recognises its duties under
the Health and Safety at Work Act
1974 to ensure, as far as is reasonably practicable,
the health, safety and welfare at
work of all its employees and, in addition,
the business of the Trust shall be
conducted so as to ensure that patients,
their relatives, contractors, voluntary workers, visitors and
members of the public having access
to hospital premises and facilities
are not exposed to risk to their
health and safety.
The duties of this
post may require the postholder to carry out exposure prone
invasive procedures on patients
of The University Hospitals of
Leicester NHS Trust. In order to protect its patients
from acquiring blood borne viral infections from staff it is required that the following conditions be met before appointment to the post is made:
1.
The candidate
must provide documentary evidence to the Occupational
Health Service as part of pre-employment
screening
of immunity to Hepatitis B infection,
either by immunisation or natural
immunity. If the candidate
is non-immune The University
Hospitals of Leicester NHS Trust will require a blood test to be carried out
for Hepatitis B antigen before appointment. The
duties of this post are such that no
Hepatitis B antigen positive candidate
could be appointed.
2. Any appointee
who is not immune to Hepatitis B, but is currently carrying
out exposure prone invasive procedures, accepts that regular checks on Hepatitis B markers may need
to be carried out by the Occupational
Health Service appointed by The University Hospitals of Leicester NHS Trust.
The tests can
be arranged, if the candidate agrees, in
confidence
through a local consultant microbiologist or the Occupational Health physician. All costs for testing
will be borne by The University Hospitals of Leicester NHS Trust.
