The post holder
will be assessed and placed on the appropriate stage of the training programme
by the Local training Committee.
Rotation will be at regular intervals into training slots offered by
hospitals participating in the Leicestershire (South Trent) Cardiology/General
Medicine Training Programme, taking into account individual experience,
preference and individual needs.
Training in both Cardiology and General (Internal) Medicine, covering
the recommended curriculum for training will be offered. Flexibility,
understanding and co-operation is expected from the post holder.
This post is
recognised for training by the Royal College of Physicians. The Postgraduate Dean has confirmed that this
post has the required educational and staffing approval.

The Glenfield Hospital,
as one of England’s newer teaching hospitals, is a 500 bedded centre of medical
excellence for clinical services, teaching and research. As one of the three teaching hospitals in
Leicester, it serves and is served by the University of Leicester Medical
School and pays particular attention to the Undergraduate teaching of medical
students.
The Glenfield Hospital has, year on year, increased
the number of patients treated, introduced service developments and expanded
the range of treatments available. We
have also gained an excellent reputation for the quality of services we provide
as recognised by the results in the Quality Health Patients’ Satisfaction
Survey as well as achieving full accreditation by the Health Quality Service
(formally known as the King’s Fund).
Glenfield Hospital provides a comprehensive range

THE WORK OF THE
DEPARTMENT:
The department of Cardiology provides specialised
services for the investigation and treatment of patients with cardiac
conditions from Leicestershire, Derbyshire, Nottinghamshire, Mansfield,
Northamptonshire and the southern half of Lincolnshire. The work of the
department involves all aspects of Adult and Paediatric Cardiology. A full
range of investigations are performed including coronary angiography,
trans-oesophageal echo-cardiology, exercise stress testing, ambulatory, stress
echo, ECG monitoring, and radio nuclear studies. Coronary angioplasty/stenting
are routinely performed with rapid treatment for those with acute cardiac
conditions. The Unit is the Regional centre for pacemakers and for the
management of arrhythmias including electro-physiological studies, radio
frequency ablation and internal defibrillator implantation.

The Department of Cardiovascular Sciences is one of the major
Departments of the Leicester Medical School. It has a national and
international reputation in cardiovascular research and in the 2001 RAE
exercise was graded 5. There are strong clinical and academic links between the
Department and the Cardio-Respiratory Directorate of UHL NHS Trust. The
Department hosts a British Heart Foundation Chair in Cardiology. A major strength
of the Department is the integration of clinical and basic research. The
Department has excellent research and laboratory facilities at Glenfield
Hospital and at the Leicester Royal Infirmary. There are also advanced plans
for building a £20 million state-of-the-art Cardiovascular Research Centre on
the Glenfield site as part of the PATHWAY project.
Research within the
Department in conduced through Research Groups, which have strong interactions
with each other, Departments in the University (especially Genetics, Cell
Physiology and Pharmacology, Biochemistry and Health Sciences) and relevant
Departments within UHL NHS Trust. There are strong collaborations with groups

DUTIES
The duties are in Adult Cardiology.
The Specialist Registrar is expected to participate in a
minimum of two out-patient clinics per week at Glenfield Hospital. He/she will be trained to perform Exercise
Testing Echocardiographic procedures, cardiac catheterisation and coronary
angiography and pacing. Experience will
be given in coronary angioplasty and electrophysiological studies.
Occasional attendance may be required in out-patients in
other District General Hospitals elsewhere in the southern half of the Trent

Teaching
The Specialist Registrar will be expected to attend
regular Thursday evening combined Cardiothoracic conferences, the monthly
afternoon audit meeting, the Tuesday lunchtime meetings of the Unit and will
also be expected to participate in lectures to colleagues at the Glenfield
Hospital with whom close academic ties are maintained. He/she will give occasional lectures to
nursing staff at Glenfield Hospital and to nursing staff from other hospitals
on the Intensive Care course, and will participate in the general training and
supervision of the SHOs and other junior medical staff in Cardiology and in the
teaching of medical students and physiological measurements technicians.
Research
The Specialist Registrar is expected to engage in
research projects to be arranged with the Consultants.
Administration
The Specialist Registrar will be expected to arrange
rotas of the junior doctors on-call, their out-patient clinic rotas and
attendance in the Catheter Room, to dictate out-patients' letters and organise
the dictation of discharge summaries of in-patients and to report on
electrocardiograms (GP direct referral service).
STUDY
AND TRAINING
There are medical library facilities at Glenfield
Hospital. The librarian is able to
obtain on loan books and journals not available in the libraries. A newly opened Postgraduate Education Centre
provides library facilities and an active programme of educational meetings.
There is an extensive Medical School library in the
Clinical Sciences Building at the Leicester Royal Infirmary.
There are Postgraduate Medical Centres at Glenfield and
at the Leicester Royal Infirmary. Lectures
on all topics are held regularly and this centre is actively concerned in the
vocational training of General Practitioners in the District. There are weekly postgraduate clinical
presentations or lectures.
Study Leave
This is granted in accordance with the recommended
allowance subject to the maintenance of the service and to prior formal
approval by the appropriate Unit.

The Glenfield Hospital is 2½ miles from the main city
centre and has ample parking and is well served by public transport.
GENERAL
INFORMATION
The appointee will work for one of the Cardiologist
firms in rotation as part of their training .
Rotation will dependant on the needs of the Specialist Registrar and the
availability of posts. Specific needs
and training will be decided following discussion with the Programme Director
(Dr Gershlick). the post is approved for
the purpose of professional training to obtain a CCST in General Internal
Medicine and Cardiology in the first instance.
THE LEICESTER ROYAL INFIRMARY
Infirmary
Close, Leicester, LE1 5WW
The Leicester Royal Infirmary is an
1171-bedded acute hospital with beds for
each of the main specialities and contains the only Accident and Emergency Unit
in the District. The hospital is
currently undergoing a development programme to provide additional facilities
and an expansion of existing services.
There is a suite of 16
operating theatres with a 24 hour recovery area, and in addition there are 3
other outlying theatres.
The Sandringham Building
which opened in 1980, provides Pathology, Medical Physics and Physiological
Measurement departments, and separate, purpose built laboratories for
Histopathology, Chemical Pathology and the Public Health Laboratory Service.
Medical Physics and
Physiological Measurement occupy two floors, with accommodation for Radio
Isotopes, E.C.G. diagnostic reporting service, E.E.G., E.N.T. measurements and
many special procedures.
The Robert Kilpatrick
Clinical Sciences Building of the University of Leicester is situated on the
Royal Infirmary site and provides offices, including the Postgraduate Dean’s
Office, and research laboratories for the greater part of the University
Departments of Cardiovascular Sciences, Oncology, Obstetrics and Gynaecology,
Paediatrics, Community Health and Pathology.
The Windsor
Building opened in November, 1992, includes acute wards, The Children’s
Hospital, Integrated Medicine wards, Haematology and Bone marrow transplant
wards, X-Ray, Pharmacy, Mortuary, Medical Illustration, Ophthalmic Outpatients,
Child Development Centre, Staff Changing, and Kitchen/Dining area.
The
Osbourne Building opened in October 1997.
It provides an integrated service including the Departments of
Haematology and Oncology. New
radiotherapy equipment has been installed and outpatient, inpatient and hostel
facilities are available within the building.
Regular meetings and lectures by invited speakers take place in the
seminar suite within the Osbourne Building.
Obstetric
and all other women’s and neonatal services have been combined into the Women’s
Hospital which opened in 1997. The
centralisation of all these services is designed to improve the provision of
women’s health in Leicestershire.

The Professorial Unit’s clinical interests are General
Cardiovascular Medicine and Clinical Pharmacology and Therapeutics with
clinical responsibility for the Cardiovascular Unit (CVU) on the Royal
Infirmary site. The special interest is
Cardiovascular Disease which includes specific responsibility for the Coronary
Care Unit and administration of the Non-invasive Cardiac Investigation Service at
the Leicester Royal Infirmary (echocardiography, Holter monitoring and exercise

Non-invasive
Cardiology Investigations
The Specialist Registrar will be expected to be involved
with reporting of the non-invasive cardiology investigations at the Leicester
Royal Infirmary via the ECG Department.
This will principally relate to over seeing the reporting on a weekly
basis 24 hour Holter monitoring tests and depending upon level of expertise,
review of urgent echocardiography reports provided by the physiological
measurement technicians. (Opportunities
for training in these reporting techniques are available within the Cardiology
Services Department). There may be opportunities for training in stress
echocardiography for experienced SpRs.
The
Cardiovascular Unit (CVU)
The CVU comprises a 9 bedded Coronary Care Unit
associated with a 12 bedded step down
unit with all modern facilities. These
include bedside and telemetric monitoring, temporary pacing facility, exercise
testing and Echocardiography. Consultant responsibilities for the running of
the unit are split between the 5 academic consultants in the cardiovascular
medicine team at the Royal Infirmary.
There is a Senior House Officer (ST2 trainee) for the Coronary Care Unit
and the Specialist Registrar Cardiology trainees provide intermediate cover on
a rota basis. Supervision of in-patient
exercise testing will be required on a rota basis with the CVU SHO staff.
ON-CALL/EMERGENCY
DUTY ROSTER:-
The Specialist Registrar will share on-call duties
(including prospective cover of colleagues on annual and study leave) with the
other Specialist Registrars.
TEACHING,
POSTGRADUATE DUTIES ETC:
Undergraduate medical students will be attached to the
firm during the year. It is expected
that the Specialist Registrar will assist with teaching the undergraduates.
POSTGRADUATE
An MRCP training programme operates at the Leicester
Royal Infirmary at various times in the year and it is usual for the Specialist
Registrars to assist in this teaching.
RESEARCH
Research is actively encouraged and opportunities are
available according to the interests of the individual.

Transthoracic echocardiography
We have 4 echo machines –
ATL x2, Cypress and SonoSite - and provide a comprehensive range of echo
services including stress echo and GP Direct Access echo. Specialist Registrars
are encouraged to obtain BSE Accreditation in Adult Echocardiography during
their attachment.
Transoesophageal echocardiography
We undertake elective and
urgent TOE studies and full training is provided in TOE.
Nuclear cardiology
We have a
state-of-the-art gamma camera, installed in 2005, and provide a full nuclear
cardiology service.
Cardiac MRI
Grantham & District
Hospital is one of the UK’s only DGHs to provide a cardiac MRI service. We have
an on-site Siemens Avanto 1.5T scanner with an Advanced Cardiac Package.
Training in cardiac MRI is offered to SpRs with an interest in non-invasive
imaging.

a weekly basis at Lincoln County Hospital.
DUTIES
OF THE POST
We endeavour to provide a
comprehensive grounding in cardiology and general (internal) medicine for our
Specialist Registrars. We have a particularly strong interest in non-invasive
cardiac imaging, and this post offers opportunities to train in
echocardiography (TTE, TOE and stress echo) and also in nuclear cardiology and
cardiac MRI. In addition, the SpR is able to undertake regular coronary
angiography sessions at Lincoln County Hospital.
The SpR participates in the on-call rota. He/she will give urgent opinions to the
non-medical specialties and supervise the House Officer and Senior House
Officer who cover the Emergency Assessment Unit. The SpR attends one
consultant-led ward round each week and also leads a ward round of their own
each week.
The
SpR undertakes two cardiology clinics each week. SpRs also have the option of
gaining experience in the Heart Failure Clinic if they wish to do so.

STUDY
AND TRAINING
There is
a strong tradition of medical education and training at Grantham & District
Hospital, and we have excellent educational facilities. We have a well-stocked
library and a modern postgraduate centre with several seminar rooms and a
lecture theatre. Regular activities include Grand Round Presentations and Guest
Lectures on a Tuesday lunchtime, SHO teaching seminars on a Wednesday
lunchtime, Audit Meeting/Journal Club on a Thursday lunchtime, and a Cardiac
Imaging Review meeting on a Friday lunchtime. Major clinical audit meetings are
held bi-monthly. Specialist Registrars are also encouraged to participate in
providing teaching for the department.
Northampton General Hospital is a large general hospital serving a
population of around 360,000. It has 730
beds, including a 4-bed coronary care unit and 22-bed cardiology ward. Nineteen physicians, working in 7 firms, have
responsibility for acute adult general medicine. There are 4 cardiologists: Dr David Sprigings
(educational supervisor), Dr Patrick Davey, Dr Dominic Cox and Dr Helen Binns.
The 2 cardiology specialist registrars (Leicester / Oxford rotations)
are each attached to one of the general medical firms with a cardiologist. They work with all the cardiologists in the
care of in-patients and out-patients with a broad range of cardiovascular
disorders. Training is provided in clinical cardiology including
coronary care, echocardiography (transthoracic and transoesophageal), diagnostic cardiac catheterisation and
pacemaker implantation. In addition, the
SpRs also participate in the general medical take, which admits around 30
patients daily, with their firm, and the continued care of these patients.

THE HOSPITAL
Kettering
General Hospital NHS Trust is an Acute District General Hospital with 540
beds. It is a modern and purpose built
Unit with 6 Integrated Medical Wards (160beds) and an Admission Assessment Unit
(23 beds), Cardiac Suite (4 beds) with a further 6 bedded step down unit.
Adjacent to coronary care there is an ITU with (7 beds).
KGH
has introduced a Management Organisation consisting of 11 Clinical
Directorates. A Clinical Director
supported by a Divisional Director leads each Directorate.
The
other hospitals in the Kettering District are St Mary's Hospital, Kettering
(Geriatric and Mental Health), Isebrook Hospital, Wellingborough (mainly
Geriatric), Rushden Hospital, Rushden (Elderly Mental Health and Mental
Handicap Services) and Corby Community Hospital.
THE DEPARTMENT
The
Integrated Medical Directorate at KGH handles Medical admissions of all
ages. All medical emergency admissions
are referred through the 23 bedded Admission Assessment Unit.
Outpatient
Clinics are held in Kettering, Corby, Rushden and Wellingborough, the major
towns in the catchment area.
The
Directorate includes the following specialties: Cardiology, Endocrinology and
Diabetes, Gastroenterology, Respiratory Medicine, Care of the Elderly, Clinical
Haematology and Nephrology.
There
is fully equipped cardiac investigations department, a comprehensive pacing,non
– invasive imaging service including transoesophageal echocardiograms. Both
Cardiologists have invasive sessions at Glenfield Hospital, Leicester.
DUTIES OF THE POST:
Clinical:
The
Specialist Registrar will be responsible with the SHO and HO for the management
of patients admitted to the Coronary Care, Step Down and Cardiology (GIM) Ward.
He/she will be expected to attend Consultant ward rounds and outpatient clinics
and to perform personal ward rounds on the acute patients with medical
juniors. The Specialist Registrar is
responsible for the medical management of the Assessment unit on
"take" days/nights.

with arrangements for duty rotas, junior staff meetings, inpatient discharge
summaries, outpatients letters etc.

Non-Clinical Duties:
Relationships:
The General duties
involve liaison with nursing staff, Cardiographers, Radiographers,
Biochemistry, Dietitians, Physiotherapists and Social Workers forming part of
the integrated clinical care team.
Co-operation and
communication with the Directorate Management team to ensure the provision of a
high standard of service to patients.
Undergraduate Teaching:
Undergraduate medical students
are attached to the medical firms of this rotation and it is expected that the
Specialist Registrar will assist with their teaching.
Research/Audit:
Various projects are
ongoing in the department. The
Specialist Registrar is actively encouraged to take part in these.
On-Call
When
on-call the Specialist Registrar will be responsible for the co-ordination of
the activities of the entire resident medical team and for the distribution of
duties between the members of that team to cover all duties of the emergency
"take" and cover of existing inpatients.
RECOMMENDED OFF- DUTY AND
COVER ARRANGEMENTS
In
accordance with the terms and conditions of service of the Hospital Medical and
Dental Staff (England and Wales) and as recommended in Health Circular DS
378/75, the Registrar will:
(a)
Be expected in the normal run of his/her
duties and within his/her contract and job description, to cover for the
occasional brief absence of colleagues as far as practicable.
(b)
For those contracts which include prospective cover,
be expected to provide on-call cover for the annual/study leave of colleagues,
providing always that the resulting increase in duties is reasonable in the
circumstances.
(c) Be
expected to perform duties in occasional emergencies and unforeseen
circumstances without additional remuneration.
The
junior doctor accepts that he/she will perform duties under these provisions,
at the request of the appropriate Consultant, in conjunction, where
practicable, with his/her colleagues, both senior and junior. Although it has been agreed between the
professions and the Department that they will perform such duties, the Secretary
of State does stress that additional commitments arising under this sub-section
are exceptional, and in particular, that juniors should not be required to
undertake work of this kind for prolonged periods or on a regular basis.
Where it is not
practicable to obtain an external locum any doctor may be requested to
undertake some out of hours duties, for which extra payment will be made. Claim forms relating to payment for this
work, outside normal working hours, are available form the Human Resources Department.
The Specialist
Registrar is a member of the "crash team" when on-call and therefore
must attend an ACLS course held in Kettering as soon as possible after
starting, unless he/she recently attended such a course.

Note: Because prospective
colleague cover is provided it is necessary for the Specialist Registrars to
discuss together, at the beginning of their appointment, their plans for
holidays so that these can be arranged at a time when cover can be provided.
STUDY AND TRAINING
Study
leave may be available in line with Department policy and must be planned in
advance.
There
is a modern Postgraduate Centre with a large lecture hall, a seminar room and a
well stocked library with audiovisual facilities. There is a full time librarian and also a
Postgraduate Manager. The librarian is
able to obtain on loan books and journals not available in the library. Lectures on all topics are held regularly.

Centre at 1330 hrs.

The
Specialist Registrar is expected to take an active part in most of these.
RESIDENT STATUS
The
post is classed as voluntary resident and you will be required when on-call to
stay overnight on the hospital site.
On-call facilities will be provided free of charge.
Single
accommodation is normally available and you will be entitled to an abatement of
65% on resident charges. Married
accommodation at full rental charge is likely to be available if required.
FUTURE CHANGES
It
may be necessary to review and possibly rearrange duties. In such a case any changes will be negotiated
with the junior doctors concerned.

This is a modern Acute General Hospital with 617 beds
covering the usual range of Acute Medical and Surgical specialties. There are 100 Adult Acute Medical beds; and a
further 70 Elderly Care rehabilitation beds.
There is further provision of rheumatology, haematology, oncology and
dermatology and there are 4 coronary care beds attached to the HDU/ICU.
Pilgrim Hospital has recently merged with Lincoln and
Louth NHS Trust and Grantham to form the United Lincolnshire Hospitals Acute
Trust covering a catchment area of 670,000.
The catchment population for Pilgrim Hospital is approximately 220,000.
There are excellent diagnostic services incluidng MRI,
Spiral CT, isotope scanning, a full range of cardiac exercise testing,
echocardiography, ambulatorty monitoring, tilt testing and access to other
investigations.

DUTIES
OF THE POST
The Specialist Registrar spends 6 months with Drs Perry
and Fairman, and 6 months with Drs Morley-Davies and Nyman. During each attachment the Specialist
Registrar is seen as a resource for other junior medical staff for both
Consultants. The Specialist Registrar is
expected to cover absences of wither Consultant on annual or study leave.
The Specialist Registrar participates in the on-call
rota. He/she will give urgent opinions
to the non-medical specialties and supervise the House Officer and Senior House
Officer who cover the Medical Admissions Unit.
The Specialist Registrar is required to review all patients seen by the
House Officer and to review patients when requested by the Senior House
Officer. The Specialist Registrar
participates on the cardiac arrest team but has protected sleep after
midnight. He/she will also be required
to do evening and/or post-take ward rounds.
Attendance will be required in the Out-patient
Department to assist with the out-patient clinics and there will be at least
one session per week in echocardiography with Dr Nyman as well as
opportunities to be involved with other non-invasive cardiology investigations.

General Information
The southern Derbyshire Acute Hospitals NHS Trust
incorporates the Derbyshire Royal Infirmary and the Derby City General
Hospital. We are a Department of Health
rated three-star Acute Trust providing a wide range of general medical and
surgical care in addition to accident and emergency services. We have a total of 1,147 beds across 44 wards
and serve a population of over ½ million people throughout Southern Derbyshire.
The trust employs 5,500 staff, from doctors and
nurses to housekeepers and porters, with an annual budget of around £300
million. This year our Trust will see
and treat around 600,000 people as inpatients, outpatients, emergency patients
and daycases.
The trust is currently working with other local
health care organisations to radically reshape health services in Southern
Derbyshire. This includes the proposal
to develop the current Derby City Hospital site to become the main acute
hospital in Derby, which will provide the very latest in modern healthcare. This will be the biggest single investment in
